Pre-Tooth Whitening Checklist: Cost of Invisalign and the Importance of Treating Gingivitis

The Pre-Tooth Whitening Gingival Quarantine is a 7-day home-care protocol designed to remove harmful bacteria from the mouth and treat gingivitis before introducing oxidizing agents into Invisalign trays for tooth whitening. At Enamel Dentistry Manor, our protocol utilizes prescription-strength chlorhexidine gluconate, 70 PSI water flossing, and Invisalign tray replacement to ensure your gums are healthy and infection-free before you begin tooth whitening.

  • Sterilization Window: A 7-day gingival quarantine is suggested to allow ulcerated gum tissue to heal before application of peroxide whitening gel.
  • Water Flossing: Water flossing can benefit gingival health through alteration of microbial composition and preventing overgrowth of pathogenic oral bacteria [1] (Xu et al., 2023). Medium-pressure (70psi) water flossing disrupts bacterial colonies of Streptococcus mutans (S. mutans)
  • Dispose of your Old Trays: Old Invisalign trays can harbor bacteria. They should be discarded when you are ready for your new tray. This prevents the reinfection of your gums during tooth whitening.
  • Investment Protection: This protocol secures the results of your ~$5,000 Invisalign treatment.

You have invested nearly $5,000 to achieve a straight smile. You are in the home stretch and ready to add that professional-grade whitening gel to your trays. But if you notice even a tiny amount of blood in the sink when you brush your teeth, your smile is not ready for whitening. Applying whitening gel to bleeding gums can be painful. If you are experiencing actively inflamed gums, simultaneous teeth whitening can allow chemicals to enter your   bloodstream. At Enamel Dentistry Manor, we prioritize your oral health. By providing a 7-day roadmap to move your gums from red and inflamed to healthy and pink, so your whitening is safe, effective, and painless.

The Pre-Tooth Whitening Gingival Quarantine is a specialized healing phase for your gums. Most online resources suggest healing your gums prior to whitening. We take it further. This is a biological reset for your oral health. The accumulation of bacteria on the teeth is considered a primary cause of many oral health conditions, including gingivitis [2] (Dani et al., 2016). During Invisalign treatment, your trays can become a breeding ground for S. mutans, the bacteria responsible for plaque and gingivitis. If not effectively treated, gingivitis can progress into periodontitis. If you introduce powerful oxidizing agents such as whitening gel while these bacteria are active, you risk severe gingival inflammation. The quarantine disrupts the bacterial lifecycle, allowing your gum tissue to tighten and seal around the neck of each tooth.

Before you brighten your smile, our team suggests you follow our clinical guide for one week prior to tooth whitening:

  • Chlorhexidine Oral Rinse: Chlorhexidine is used in dentistry for its antiseptic properties and efficacy in inhibiting biofilm and plaque accumulation [3] (Deus et al., 2022). Use a 0.12% Chlorhexidine rinse twice daily. This is a gold standard for killing the bacteria that cause bleeding gums.
  • The 70 PSI Water Floss: Set your water flosser to 70 PSI (usually the medium-high setting). This specific pressure is required to flush out the deep pockets where S. mutans reside.
  • Aligner Sanitization: If you are at the end of a tray cycle, discard your old aligners. Old plastic can harbor microscopic bacterial biofilms that a toothbrush cannot reach. Start your whitening with a fresh, sterile tray.
  • Alkaline Diet Shift: For these 7 days, avoid acidic coffee and sodas. A neutral pH in your saliva helps your gums heal 30% faster.

1. Why do I have to throw away my old Invisalign trays before I start whitening?

Old trays develop a microscopic bacterial biofilm. If you put whitening gel into a dirty tray, the peroxide reacts with the bacteria instead of your teeth. Starting with a fresh tray ensures the whitening gel works only on your enamel.

2. Is 70 PSI on a water flosser safe for my gums?

Yes, if your gums are healthy. During the quarantine, 70 PSI is the clinical threshold needed to physically remove bacteria from the gingival sulcus. If it causes significant pain, it may be a sign that we need to treat your infection professionally before you whiten your teeth.

3. What happens if I skip the 7-day checklist and whiten anyway?

Skipping the preparation phase may lead to acute tooth sensitivity, which can be painful. It can also cause your gums to turn white and peel (chemical necrosis). Allowing your gums to stabilize for 7 days prior to tooth whitening helps to prevent these unpleasant experiences.
