  • Improved periodontal hygiene leads to successful occlusion correction. In the past, dental providers considered active and historical gum disease a contraindication for orthodontic treatment. However, the primary risk is not the movement of the teeth with reduced bone support itself, but the direction and intensity of the force applied to the teeth with reduced bone support.   • Patient Presentation:
    • A mid-30s patient presented with a clinical history of stage 2 periodontitis and moderate crowding of the teeth. Their primary goal was to maintain the health of the remaining bone structure while improving the aesthetics of their smile for high-profile local events.
  • Underlying Complications: 
    • 3-4mm pockets of attachment loss across the mandibular anterior teeth were revealed upon clinical evaluation. The patient required frequent deep cleanings, including teeth scaling and root planing, and traditional braces were ruled out. 
  • Treatment Protocol
    • Dr. Hardik used iTero Element 5D to create a 3-dimensional replica of the patient’s mouth. AI software then planned Invisalign trays in a way that tooth movement was staged so that only two teeth were moved at any given time. This planned movement ensured that teeth with the most bone loss never experienced excessive strain.
  • Results
    • After a year, the patient successfully achieved full alignment of the teeth, and more importantly, follow-up imaging demonstrated no additional bone loss. Invisalign trays were able to be removed, allowing the patient to maintain their strict hygiene routine and floss effectively throughout treatment.

Alveolar bone supports the tooth. In patients with periodontitis, alveolar bone loss occurs from prolonged inflammation of the gums caused by periodontopathic bacteria [3] (Usui et al., 2021). When alveolar bone loss occurs in patients with periodontitis, successful tooth movement becomes more difficult. Standard orthodontic pressure from traditional braces moves the force toward the root of the tooth. This angles the tooth out of the bone instead of moving it with the jaw. At our Lantana office, Dr. Hardik evaluates the health of the periodontal ligament (PDL) before any treatment begins. Since bone loss reduces the supporting bone tissue, tooth movement must be performed at a low pressure and slow speed. If tooth movement is too fast or at high pressures, periodontitis can worsen.
